Job Description:
Our Hospice RN Case Managers are vital to providing
compassionate, holistic care to patients and families.
They conduct comprehensive assessments, develop and reevaluate care
plans, and administer medications per physician instructions.
They counsel patients and families, maintain accurate clinical
documentation, and collaborate with interdisciplinary teams to meet
medical, emotional, and spiritual needs.
Candidates must have an RN license, at least 1 year of nursing
experience (3+ preferred), and knowledge of hospice care.
