Job Description:

We are seeking Advanced Practice Providers with experience in acute care to join our growing in-home care team. As an Advanced Practice Provider, you will work alongside a clinical care team to provide comprehensive health assessments, diagnosis, treatment, and outcome management for patients of all ages. This role involves delivering care in the patient's home through innovative in-home care service lines. You will also have the opportunity to perform therapeutic interventions, educate patients and families, and effectively communicate with all individuals involved in the patient's care. Requirements:

• Minimum of two years of experience as an APP in relevant areas
• Graduate with an advanced degree from an accredited institution
• Current unrestricted state license as a Nurse Practitioner or Physician Assistant
• Nationally board certified, current
• Current BLS required, ACLS certification preferred
• Prescriptive authority and DEA
• Valid driver's license with clean driving record
• Ability to lift and carry equipment up to 50 pounds
• Flexible to work evenings, weekends, and holidays, as needed What We Offer:
