Join our elite team of healthcare professionals, providing the Right Care, at the Right Time, in the Right Place.Job Description:The Home Health Aide provides personal health care, related services and additional tasks as assigned to the client in the home; assists in providing a safe and hygienic environment, works cooperatively with patient/family and shares observations and problems with the supervisor.JOB REQUIREMENTS:EDUCATION:Requires reading, writing, and math skills normally obtained from a high school diploma or GED.State-established or other Home Health Aide training program that meets the requirements of 42 CFR -484.36 and a competency evaluation program, orState licensure program that meets the requirements of 42 CFR -484.36 or within the past 24 months, orA competency evaluation program or State licensure program that meets the requirement of 42 CFR -484.36 within the past 24 months.Otherwise, is required to successfully complete a Home Health Aide training and competency evaluation program prior to providing direct patient care.EXPERIENCE:Minimum of one (1) year of experience as a Home Health Aide or Nursing Assistant. Experience in a hospital, nursing home or home health agency preferred. ---CERTIFICATION/LICENSURE: Certified Home Health Aide (CHHA), or National Association for Home Care and Hospice (NAHS) is required.SKILLS:Ability to work an on-call weekend rotation to meet the needs of patient 24/7Ability to read and write and carry out directions in EnglishDemonstrates a sympathetic attitude toward the care of the sick and desires to provide care in the homeSelf-motivatedDemonstrates effective time management skillsTRANSPORTATION: Reliable transportation.TRAVEL: 50% travel.HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing.KEY ACCOUNTABILITIES: ---Ensures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team. ---Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care. ---Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. ---Verifies assignment, reviews care plan and calls patients daily before the visit occurs. ---Promotes positive, supportive, respectful communication to patient, family, and other employees. ---Introduces self, communicates in clear, friendly manner, and explains procedures prior to performing tasks. ---Provides an environment which promotes respect for patient, privacy, and property.
