an acute setting, and 1 year of case management and quality

Provides clinical operational oversight, education and support for
the post acute skilled nursing facility programs within an assigned
region. Provides subject matter expertise, consultation, problem
solving, critical thinking and education to physicians, hospital
leaders, referral sources and skilled nursing facilities. Position
is located in an assigned territory and hospital market and makes
regular weekly visits to post acute providers to monitor the
effectiveness of the program as well as to assist with the
implementation of identified processes that improve the transition
of patients across the continuum of post acute services. Resource
for leadership teams at the hospital and network post acute
provider administrative teams. Manages and makes recommendations
for the operational processes that insure cost effective and high
quality services for the Advocate Aurora patients.
Serves as the liaison between the hospital and the post acute
network (PAN) skilled nursing facilities. Manage and facilitate
communication between the inpatient care managers, external care
providers and outpatient care managers for high risk patients
transitioning in and out of the acute care setting and post acute
programs. Assist the transition of high risk patients to post acute
programs and eventually back to their primary care physician (PCP)
Monitors, collects and analyzes data to evaluate the effectiveness,
safety and quality of patient care in the post acute care network
Addresses questions and concerns by providers, patients and
families regarding post acute programs in coordination with other
care teams. Acts as a conduit for problem solving, resolution and
