We Are Hiring:
AMITA Health Home Care is hiring a full-time,
day shift, LPN to join our multidisciplinary home health care team.
This position will provide you autonomy as you will manage your
daily visit schedule independently.
Work Hours: Monday thru Friday
rotating weekend and holidays.
You will receive a laptop or android tablet and
iPhone, and your supplies will be delivered directly to the patient
with no office pick-up required. This position will offer you
support through our well-constructed onboarding and orientation

What You Will Do:
Provide care to patients under the
direction of a registered nurse or physician, functioning within
scope of license.
What You Will Need:
- Deliver care to assigned clients and performs direct care in
accordance with established plan of care.
- Deliver safe care at all times that is in line with physician
orders, home health policy and established nursing care
- Maintain communication regarding patient progress with
physician, supervisor, staff and family members as appropriate to
- Report patient-related observations to supervising RN on a
- Administer medications and observe patients for adverse
reactions to medications or treatments.
- Provide documentation that follows the established treatment
plan, supports coordination of patient care, meets regulatory
requirement and assures reimbursement.
Licensure / Certification /
- BLS Provider credentialed from the American Heart Association
(AHA) obtained prior to hire date or job transfer date
- Drivers License credentialed from the Illinois Department of
Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer
- Practical/Vocational Nurse credentialed from the Illinois
Department of Financial and Professional Regulation obtained prior
to hire date or job transfer date required.
