LPN HOME HEALTHLOCATION: NEAR NORTH CITYHOURS: DAYSThis position is also eligible for up to a $5000.00 sign on bonus! You must be employed in the exact position at time of payout. Internal team members are not eligible for this sign on bonus at this time.Full TimeMust be able to work 1 summer and 1 winter holiday and rotate weekendsMust be able to travel north city and north suburbsHow You Will Make A DifferenceResponsible to the Clinical Manager for instructions on case management and accountable to administration of patient care.Will carry out selected aspects of the patient care activities to a group of patients in a manner consistent with the philosophy of Advocate Health at Home.Assure and improve quality of care for patients.Facilitate patient and family rights to receive quality cost effective care.Position RequirementsLicense in the State of Illinois2 years of med/surg clinical experienceAbility to learn and use a computer based scheduling and documentation system.Ability to communicate professionally both verbally and through written reports.Good interpersonal skills.
