Classification: Hourly/Non-Exempt Who we are: Symetria Recovery
provides comprehensive substance abuse treatment with a focus on
Medication Assisted Treatment (MAT) in a welcoming outpatient
setting. Unlimited clinical services are available to our patients
under our bundled-service delivery model. Key services delivered
through our Opioid Treatment Program (OTP) licensure include MAT,
medically monitored psychiatric care, individual therapy, group
therapy (IOP), 6-day-per-week nursing care, and ongoing aftercare.
Most of our patients being provided medication receive
Buprenorphine products, but they can also receive Methadone,
injectables (Vivitrol, Sublocade), and any other FDA approved
medication for addiction. Symetria is an innovative and growing

team Job Purpose : The Licensed Practical Nurse is responsible for
dispensing any medications the clinic may utilize in the treatment
of patients in recovery. The LPN will work with the clinic staff,
Practitioner, Medical Director, and Program Director to assure
appropriate medical care of patients. Job Responsibilities :
Prepares, dispenses, and administers prescribed medications per
established schedules as ordered. Monitors patient's vital signs
for dose changes and other physical health information, maintains
accurate records reflecting data, including general information
relating to treatment results. Collects blood samples from patients
and monitors and attends to the physical needs of the patient.
Administers injectable medications, ensuring accurate prescribed
dosage and timely delivery, educating clients in management of side
effects, and obtaining pre-authorization of
prescriptions/medications from pharmacy, when needed. Administers
and reads PPD, pregnancy tests, tuberculosis screening tests, and
drug screens on patients and documents results. Maintains accurate
inventory counts of all medicines and medical supplies and ensures
adequate replacements are ordered on a timely basis, as well as
reconciling inventory of Methadone, Suboxone, and Subutex at
conclusion of dosing. Provides daily reports to Program
Director/Nurse Supervisor. Transcribes, signs, and processes MD or
PA orders and obtains requested medical information relating to
patients. Documents ---no-shows---, processes phase change request,
updates Methasoft, and inputs flags as per physician orders.
Documents and maintains accurate monthly medication records and
filing of other paperwork. Strictly adheres to LVN licensing
ethical standards and Medication Room policies and procedures Works
as a team with clinical staff and physicians Follows all Symetria
policies, state regulations, federal guidelines, and DEA
regulations. Employee is expected to perform all duties in
accordance to work schedule and as assigned. Experience, Skills
and/or Requirements Current and valid LPN license with no
restrictions and licensed within the state of assigned clinic as
LPN or RN required. 1-year experience as an LPN or RN required.
Eighteen months of education or training beyond the high school
level in Nursing or related field of study required. Graduation
from an accredited school of nursing required. CPR certified within
three months of hire. Must pass a criminal background check.
Experience in medication assisted treatment in an opiate treatment
or mental health facility setting preferred. Must be capable of
working under pressure as well as communicate, organize, and plan
effectively and efficiently. Physical Demands Receives and follows
detailed instructions. Required corrected vision and hearing to
normal range. Requires occasional bending, reaching, and
stretching. Must maintain a professional working relationship with
patients and staff. Must be able to concentrate for extended
periods of time, paying attention to detail. Must be able to
receive and follow, as well as give detailed instructions. Requires
sitting for long periods of time. Some bending, lifting, stooping,
and stretching required. Requires normal range of hearing and
eyesight.
