Job Description Independence Plus is rapidly expanding our skilled home health division in all of our service areas. We are now hiring a full-time Home Health Visit Registered Nurse to do Home Health Visits in Chicago's northern suburbs. The RN in this position plans, organizes, and directs home care services, and is experienced in nursing, with an emphasis on community health nursing. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Complete 8 Continuing Education Hours with our PAID training and Ventilator Certification JOB DESCRIPTION: Completes initial and on-going comprehensive assessments, including Outcome and Assessment Information Set (OASIS) at appropriate time levels Uses health assessment data to determine nursing diagnosis and interventions Follows and updates the plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing actions, including the patient and the family in the planning process Initiates and follows through on rehabilitative nursing procedures, including but not limited to administering medications and treatments as prescribed by the physician Counsels the patient and family in meeting nursing and related needs Communicates with community health related individuals to coordinate the care plan Provides patient care in a manner that satisfies patients and families, meets or exceeds current standards of practice, state, federal and CHAP guidelines, and follows the policies and procedures of Independence Plus, Inc.
